Bridging Community: Collaborative Efforts in Early Detection and Intervention for Alzheimer’s Disease is an innovative educational program tailored for family medicine physicians, as well as primary care practitioners—including OB/GYNs, NPs, PAs—alongside general neurologists, psychiatrists, and other members of the interprofessional healthcare team. This program empowers clinicians who play a pivotal role in identifying and managing patients at risk of early Alzheimer’s disease (AD) and cognitive impairment.
This initiative features a blended learning experience combining an enduring on-demand component with interactive in-person sessions hosted by local AAFP chapters. Participants will gain actionable insights from expert faculty, who will explore:

Dr. Cole received her medical degree from the University of South Florida in Tampa and completed her residency and fellowship training at Florida Hospital, now AdventHealth. As part of her role as director of the Geriatric Fellowship, Dr. Cole supervises medical students, residents, and geriatric fellows in the care of elderly patients in outpatient settings and nursing homes. She oversees a Comprehensive Geriatric Assessment Clinic where elder patients and their caregivers meet with an interdisciplinary team to evaluate their medical, functional, and psychosocial status.
Dr. Cole is the Orlando campus Clerkship Director for Geriatrics at the Florida State University (FSU) College of Medicine. Dr. Cole is a member of American Academy of Family Physicians, the American Geriatrics Society, and the American Medical Directors Association. She is Board Certified in Family Medicine and Geriatric Medicine and is a Certified Medical Director.

Dr. Atri is Chief Medical Officer of Banner Research (Banner Alzheimer’s Institutes, Phoenix and Tucson) and Director of the Banner Sun Health Research Institute (Sun City), AZ. He serves as Associate Director of the NIA/NIH-funded Arizona Alzheimer’s Disease Research Center (AZ-ADRC), Leader of the AZ-ADRC Clinical Core, and Co-Leader of the Biomarker Core; and on the part-time faculty at Brigham and Women’s Hospital (BWH) and Harvard Medical School (HMS), Boston, MA, USA. He is the William B. and Sheila Konar Professor of Psychiatry, Neurology, Neuroscience, and Medicine at the University of Rochester School of Medicine and Dentistry. Dr. Porsteinsson has devoted his career to the care and study of individuals with memory disorders.
